Register as a new patient at Bridge View Medical

You can only Register with us if you live within the Bridge View Medical Practice Boundary. To find out whether your address falls within this area please see the map via this link or alternatively the registration team can be contacted via email bnssg.bvmregistrations@nhs.net. 

We are using a new online service called Register with a GP Surgery. That makes it easy to register with Bridge View Medical.

Click on one of the links and fill in this quick online form to start the process.

You do not need proof of address or immigration status, ID or an NHS number.

Although you can choose a site to register with, as Bridge View Medical is one practice you may be asked to attend one of the other sites.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

All registrations must include BOTH forms below.

• Form 1 -NHS GMS1 Form 
• Form 2 -Bridge View Medical Registration and Health Information Form

We ask you to complete the health questionnaire at the time of registration. This questionnaire will include questions about lifestyle and medical history and must be completed alongside the NHS registration form in order to register.

If you wish to gain online access to your medical records, we will need to see you in person with some photo ID to verify your identity. This is due to the confidential information accessible on the online access platforms. 

Please return completed forms to us by email if you can, alternatively you can hand them in at your local surgery branch.

1. Form 1 - must be completed for all registrations
2. Form 2 - must be completed to register - ADULT
   Form 3 - must be completed to register - CHILD

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

To do this, please complete the following form from and hand into one of our Surgeries. You can collect hard copies of this from us too.

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
